First of all, manufacture a steel plate that's 3mm thick, as illustrated in the image labeled pic#2. Then, use the original carriage as a stencil to drill holes into it - you can leave three bearings intact if you prefer, but I strongly advise drilling four for optimal results. Next, print bearing gaskets, one for each bearing; these should be printed with the Heatbed positioned above them and not resting on the Y-motor. You have two options for attaching belts: you can use the original attachment or print a new one that includes a tensioner - just remember to drill holes for it in the process.
