I took dumle29's outstanding design and tweaked it significantly. I added some notches for the nuts, reduced the overall height and created a chamfer on one edge. These modifications make the entire assembly sleeker and simpler to install :-). I combined it with a VESA 75 to 100 adapter, which I might just incorporate into this design - Adding it would be quite straightforward.
