Truncus arteriosus occurs when the heart and great blood vessels develop abnormally at birth. In type I truncus arteriosus, a shortened common pulmonary trunk gives rise to both right and left pulmonary arteries, as identified by Collett and Edwards. The truncus valve is typically composed of three leaflets, with double nodules located on the posterior cusp, known as Arantii's nodules. Both coronary openings are situated in the posterior sinus, where they receive normal blood flow distribution. A distinct endothelial ridge forms below each opening, resembling a valve-like structure. The right-sided aortic arch is a notable feature of this condition, often accompanied by a ventricular septal defect that affects the membranous portion of the heart.
