This is a toothpaste cap tool designed to assist either small or elderly hands in turning the top of the toothpaste. It tapers from a 15mm to 17mm hole with small teeth that help grip the cap firmly. The teeth on this tool are relatively tiny, smaller than my nozzle could print at 0.4mm, so it was omitted, but still seems to work perfectly fine.
