You'll need: 80x20mm board, six tiny whoop female connectors, one 6s JST-XH balance plug with wires, six 2.54mm pin headers, two banana connector 4mm, one dupont female connector, some wires, and heatshrinks. Note: Moving the black jumper to the backside lets you choose the number of batteries from one up to six (see pictures; in the scheme, instead of the jumper, it's from the main port to the selected blue dot, 1 to 6). I used a bit of glue to close the top.
