This is a customized version of http://www.thingiverse.com/thing:47956, created using the Customizer! (http://www.thingiverse.com/apps/customizer/run?thing_id=47956). To create this unique design, specific options were used, including plate_width of 1, no 3rd, 4th, or 5th plates, a blank 1st plate bottom hole, and none for the 3rd plate bottom hole. Additionally, there is a toggle 2nd plate, an outlet 1st plate, and no holes on the 5th plate or the 4th and 2nd plate bottom holes.
