I was disappointed by the cost of bed rail insert nuts for my Toyota Tacoma, so I designed and built my own. It's modeled after a hammer-style nut commonly used in 3D printing applications. The hole is 0.26 inches in diameter, allowing it to be tapped with either a 5/16 or M8 screw. I fabricated mine from scrap 1/2-inch aluminum using a milling process.
