In order to save $60, I purchased a two-piece adaptor from a camera store and discovered that it was merely made of plastic. Consequently, I decided to model and print my own adaptor using the camera attachment design from nickswimsfast's Thingiverse model (http://www.thingiverse.com/thing:88093). However, I made slight adjustments to the original design for improved functionality.
