This extruder mount is a fusion of two swivel extruders from Thingiverse: http://www.thingiverse.com/thing:952549 and http://www.thingiverse.com/thing:1037361. One key modification made was adding an extra bearing, with 4 M8 washers stacked as spacers between the bearings to prevent binding when the assembly is tightened down.
