I'm using a custom SJCAM5000+ mount for my Emax Nighthawk 250 quad. Tested: works great Made from high-quality PLA material Hole diameter: 39x38mm 09/28/2015 - I designed a tilted mount with a 10° angle as requested. Unfortunately, the camera box was too tight to fit properly, so I had to drill holes in it to get it to work without using a strap for support. Please let me know if this version meets your requirements. 10/02/2015 - I've made some changes to the previous design and corrected the issues with the "too tight" version. While I haven't printed it myself, it should fit properly now. Please give it a try and let me know if everything looks good.
