I'm reworking this content, and I must admit it's a mashup of sorts, but I'm editing it from scratch since I couldn't locate the original file. I want to give credit where credit is due - I didn't come up with this entirely on my own; I simply stripped away some unnecessary elements to make it print better for lower-end printers.
