The covers for the bolts on my Winnebago RV seat belts kept falling off because they lacked any kind of retention mechanism. One of them eventually got lost, so I designed and created replacements that stay securely in place. In image #1, the cover on the left is the OEM version without any kind of retention feature. The one on the right is my version with tapered 1.0mm retention flanges added to keep it firmly attached. The #2 image shows a bolt head with the original cover missing. Image #3 demonstrates how my cover stays securely in place, thanks to its retention flanges. I recommend printing these covers in TPU so they can be easily removed if needed. This particular design fits a 30mm hole perfectly. Simply scale it in both the X and Y axes to make it fit larger or smaller holes. For example, if you want to adapt this cover for a 40mm hole, simply calculate the scale factor: 40mm / 30mm = 133.33%.
