A buddy asked me to whip up something similar for him, draw it out, slap it onto a printed sheet, then discover others had already done the same thing. Still, he shared it anyway. You can attach this design to a metal ruler measuring 12 mm wide and cut a thread M4 into it. To print, use a little negative expansion setting.
