This is the RGB2HDMI video slot bracket for the CPLD version of the Amiga 2000 slot version of the card with 3-button support. I based this on the measurements of Pete Biggs' slot bracket, but made this in Sketchup to accomodate 3 buttons. Feel free to remix this to another design for different HDMI connectors, more/less buttons, etc. Sketchup source is in the file downloads.
