I just replaced my spool holder's filament guide, which had broken off, by creating a new model that only includes the guide itself. It's held together using a single 4 mm screw and features holes for both the screw head and a square nut. Luckily, I had all these parts lying around from an old Meccano set, so it was a great solution in this situation. For added grip on the clamp part, I applied a small piece of tape to its inside surface. Printer: Printrbot Simple Metal w/ heated bed. Printed with PLA at 200 degrees.
