This is a housing for the standard PIR sensor module, similar to the one sold by Adafruit: https://www.adafruit.com/product/189. It has a 20-degree downward tilt. Three lids are available - one with no side/top obstructions, one with 5mm obstructions and one with 10mm obstructions. The obstructions can be useful in preventing detection from areas other than directly ahead and below. A typical use case for this would be to detect an approaching person as they approach a door and trigger a mechanical release.
