I needed a replacement for a chewed-out pipe stem for my old Gulden Dansk pipe, which I got for free with tobacco coupons back in the 1980s. It turned out to be pretty straightforward to do using 123D Design. This was my first try at using the Loft tool, thanks to a great tutorial on YouTube. I measured the outside of the original and put those measurements into a spreadsheet to keep track of them. I had to make an educated guess about the smoke passage (should have just cut the old one in half, but I wanted to keep it intact just in case the print didn't work out!). The original stem had a stylized GD logo on it; I didn't put too much effort into duplicating the font. Update: I removed a constriction in the smoke channel that was making it tough to get a pipe cleaner through.
