This is a 2 Gang low voltage box cover. I designed it to fit the same box that the RACO 2 Gang Square Box Cover fits. https://www.lowes.com/pd/RACO-2-Gang-Square-Metal-Electrical-Box-Cover/3129183It's perforated to allow the signal from a Shelly 1 relay module to pass through the box.Prints with PLA and requires only the same diameter screws included with the RACO cover.Holes are all 5mm.
