You can adjust the tension by turning the screw by hand. I was using it for 6 months and have 0 problems. It uses one of the spare M5 screw that comes with the 3D printer. I cut it so it doesn't stand out too much. And it is threaded on the plastic part only with the screw, you can use a tap, but I think it is not necessary. For the bearing I put a washer between the arm and the bearing and file the screw a bit because otherwise it wear the bottom part. The arm is not modify, I only took an original extruder model and modify it so you can screw the PTFE tube fitting and the M5 screw have some place to rest.
