I've successfully tested creating something from scratch using OpenSCAD (and Inkscape) to see how it works. The name tag doubles as a name-stamp on the backside. To make your own, simply change the name in the SVG file, then convert font into object, flatten Beziers and export as DXF file. You can fine-tune sizes of things in the OpenSCAD file for better results.
