Using the options to customize, make your selections with 2nd_plate_bottom_hole remaining unchosen. Keep plate_size at zero while toggling on/off 1st_plate. Leave its bottom hole blank. Choose a plate width of three. Then turn 2nd_plate on and off in order. The bottom holes for 4th and 5th plates remain unchecked as well as the top ones themselves.
