Monster Hat Direct - Klemco E3D (Ender3) direct drive + 40mm fan

A direct drive conversion for Klemco (or compatible?) E3Dv2 conversion kits, using a Microswiss dual-gear direct drive and a 40mm fan for hotend cooling, and leaving both accessory mounts free. Version 1 didn't take into account that the microswiss is a little wider than the stepper motor, and was about 0.5mm off center. V2 doesn't have that problem! Inspired by Schmerzfabrik's direct drive adapter https://www.thingiverse.com/thing:5360818, but with the drive rotated so that it won't run into linear rails. You need at least a 30mm thick drive to get enough torque with current generation steppers. If you're direct drive, you probably already have a dual-screw arrangement so that it won't sag, so you might as well just use a hefty stepper. Fan "ducts" for non-blower fans hurt airflow and cooling ability, but you don't want much of the air going down to the workpiece so it uses just a bottom guard to help with that (1/8 of an existing project). Uses remixed elements from other projects (thanks!) for the NEMA17 motor mount and airguide, credited in the built-in "remix of" section.
