Maxillary arch form is crucial to understand for optimal orthodontic treatment planning and diagnosis. In this article, we will explore a specific maxillary arch case that highlights the importance of understanding arch form. The patient presented with a severe Class III malocclusion due to a narrow maxillary arch. The upper central incisors were crowded, and the lateral incisors were severely displaced, creating a Bolton discrepancy. To address this issue, our orthodontic team decided to use a combination of fixed appliances and a palate expander. The goal was to widen the maxillary arch and create space for proper tooth alignment. The treatment plan included extraction of four premolars to allow for adequate space closure and to prevent future relapse. Additionally, a high-pull headgear was used to correct the Class III canine relationship. After 18 months of active orthodontic treatment, the patient's maxillary arch had expanded significantly, allowing for proper alignment of all teeth. The Bolton discrepancy was corrected, and the patient achieved an optimal occlusion. This case highlights the importance of understanding maxillary arch form in orthodontic diagnosis and treatment planning. A thorough analysis of the arch form can help prevent unnecessary extractions and ensure optimal space closure.
