Here's a (work in progress) auto leveling mount for an E3D hotend. Micro servo, blower, and 40mm fan are all mounted on this plate. The blower duct also included in the files. Z offset seems to be about -5.8mm. X offset is roughly -45mm while Y offset is around -5mm. My configuration.h attached for reference, I'm using Marlin 1.0.2 with the Makerfarm Pegasus 8". Note: In the picture, the servo is attached in reverse. The servo should be attached with its shaft pointed away from the hotend and nearest to the X axis rails.
