I created this model using Autodesk 123d Design and printed it with great success. WARNING! !!!!!!!!---> I've made some updates to the model, and the revised version fits better on the canon attach and focuses more accurately towards infinity. Therefore, I highly recommend printing the second model (m42_EOS_Adapter_Final.stl) as well, since it requires less filament for printing. <------!!!!!!!
