I've had to disassemble my hot end numerous times to cut away clogged PTFE tubes that slipped a little from the nozzle. It appears that a millimeter or two of slippage can cause a gap between the back of the nozzle and the heat block, allowing molten plastic to seep in and lead to under extrusion, partial or full clogging, as well as retraction issues. To prevent this, I've designed a washer that keeps the bowden tube pressed firmly against the back of the nozzle, eliminating any gaps in between. After using it for some time, I'm pleased to report that it's worked flawlessly for me, and I think sharing it could be beneficial for others who may face similar problems.
