Custom design based on Nother_Ngineer job (https://www.thingiverse.com/thing:4200973) created using OpenSCAD for toothpaste tube sizes up to 8oz and 65mm width. Printing: PETG is used at a layer height of 0.20, 25% infill, and brim enabled with no supports needed. SCAD file is included for customizing dimensions, shape, handle style, etc. Components are in one file but can be split apart in slicer software and printed separately for higher quality or different infill/etc. Assembly: The twister rod is pressed into the main body completely (past the retainer tabs). Usage: Toothpaste tube should be inserted into the twister rod from the side after at least 10% of the tube is consumed, then twist knob in your favorite direction. Remove empty tube by turning it slightly in reverse to loosen and pull out from the side; no need to unroll completely. Update 4/6/20: Round body option added (see 2nd photo). Enjoy! Use slicer software to split and print separately if wanted. Printing twister rod separately or higher resolution may reduce layer lines for less friction when removing used tube.
