Parts needed - 1x HD15 male 1x HD15 female 1x Chassis mount RCA jack Wire - Ribbon cable is easiest. Wire all appropriate RGB and ground wires, and split the sync to the RCA jack. Underside lid is a simple snap fit in place. Its a tight fit, but a dab of glue could be used as well.
