(Table saws are dangerous. All tinkering with this and the use of this are at the end users own risk.) Such a great table saw, originally made in Canada, alas no longer in production. As such, very few plate inserts can be purchased. I do make them out of wood as many do, but this seemed like a fun thing to do, and it worked out far better than I'd imagined. So for the time being, I'm using this one. Personally I printed this with a brim, because you want to go for a no warp flat as hell scenario. So trim that off once it's printed. There's tabs cut out to allow the plate to recess. Into the 1mm holes you should be able to work in a longer grub screws to act as levelers.. Likewise, on the user end there's a hole to accept a screw or threaded bolt to snug up any slop. I found with this, and because I also use suction at the table, I don't require any additional hold downs on the output side where there can sometimes be a little tab. You could certainly drill and put a screw there too if need be, but the snug screw works so well you may end up with a stuck insert plate at that point... The side to side play on mine was non existent. These slots are machined to pretty exact sizes, so you should be in a similar place there. After it prints, you'll need to carefully stick it to the back of the old insert, and slowly raise up the blade to cut out your anti tear out zero clearance fun. Raise the blade all the way up to cut it through. Turn off, and set about installing your level screws (grub). I did leave a slot at the end for a splitter. I suggest you use one. You'll need to align and re-cut that to size for your specific saw, as this can't ever line up perfectly. I added it on the file to re-enforce the edges to allow the cut, not to be good to go and aligned. That could never be possible in a cut to size application.
