This is a straightforward Velcro-attached dash mount designed specifically for a Galaxy S4 inside an Otterbox Commuter case. The mount allows room for similar sized cases, and it comes with an additional dash mount for the iTrip FM transmitter seen in the image. Additionally, Solidworks files are available for download.
