You should have noticed when the screw over the fan is tightened, the fan starts making noise because the blade touches the heatsink. This happens due to a lack of support on the top side. This block will solve this problem and allow the fan to blow directly over the plastic part and barrel. Since the heatsink isn't touching all plastic parts, it's not efficient. You'll need: 2 - M3 x 16 screws 2 - M3 washers 2 - M3 hex nuts The other screws are already in your extruder. If your printer is not perfectly calibrated, use a 5 mm drill to enlarge the holes.
