Here's an extruder I designed that I based off of the SeeMeCNC EZR Struder. NOTE: Use supports and if you're using the Cura slicer, place support blockers exactly where I showed in one of the screenshots above or else removing supports will be a huge hassle. Also, 20% infill works best for the extruder, but I would suggest 80%-99% for the slide due to it being really thin.
