In the attached file, you'll find a mesh where the gem would be placed. There's no reason why you can't design your own pattern and set a real gem on the ring. Any pattern can be added to the ring, and with Fluid Designer for 3D Printing, there are many options available. I use curves instead of meshes to create the ring. You need to take care when working with intersecting parts, but Netfabb Basic will usually sort out any issues. In this example, no issues arose (which is quite rare).
