This models is easy to print and is essentially a frame for the filter cloth with rivets to hold the cloth in place. You will need and elastic strap 1cm wide to attach it to your face. Print it in PLA or better yet some kind of flexible filament. Heat the frame with a heat gun or a hair dryer set to high and mold it to your face. Glue the folds together. Cut the cloth in line with the template and stab the holes for the rivets. There is a PDF A4 size template if you have 1mm thick plasticard. Principle is of heat forming it is the same.
