I strongly suspect a Varus shim is needed for my left foot since my knee shifts inward on the pedal stroke. A prominent bike manufacturer sells one for €15 plus shipping, but that would take forever these days, so I used it as inspiration to design my own. I uploaded both the 2D SVG and the OpenSCAD file I used to create the angled surface. Only the Varus version is provided (big toe higher than others), allowing you to customize to your needs (Varus/Valgus), shoe size, or different angle. The inward side has fewer holes. Scale the width to match your insole's forefoot width; mine is a size 45 Shimano shoe with a 94mm wide insole, so it's scaled accordingly. Recommended print settings are: 0.1mm layer height for smoothness and 100% infill to prevent collapse. Print face down, with the insole touching the smooth side facing outwards to prevent tearing. I suggest printing in TPU, but an initial prototype made from ABS works well enough so far.
