This is a custom version of the CR-10 Ultimate Leveling Knob Attachment from Valstrath, shrunk down to fit the S5 model. The installation procedure remains identical to the original design: Remove the bed knob completely from the bed. You'll notice that the printed knob has a circular collar, while the bed knob features a round lip. Insert the collar of the printed knob into the lip on the bed knob and then screw it back on with the modified side facing upwards. This will prevent the wheel from coming loose during a print job. It's also possible to use this attachment with the S4 model.
