Computermate 5.25" Disk storage organizer insert

I use two old Computermate 5.25" disk keepers to store both floppies and CDs. The original organizer inserts have broken over time, so I've come up with a quick re-design for one of those inserts. This model is a bit tight, but it works fine for now - I might need to shrink it slightly later on. No special printing settings are needed here; the design is entirely flat.
