I discovered an excellent card holder box at http://www.thingiverse.com/thing:30592, which I highly recommend. To improve accessibility, I modified the design by adding a front-facing mirror and attaching a fastenable lid with a rubber band. Additionally, I increased the height of the holder slightly and reduced the thickness of the base for better fit. Printing at a .5 nozzle, .4 layer height, and .3 infill density yielded satisfactory results.
