Thread the elbows into the light base, then JB weld them on the inside so that they stay aimed down. Same with the extension into the bottom of the light base and to the floor flange. Pay attention to orientation so the screw holes on the floor flange are lined up with the elbows pointed down. The screw holes on the floor flange are on a 2.125" bolting circle diameter. Wire it up so that the top bulb is on the blue wire of the switch, and the four flame lights are on the red wire. Have them all share a return wire - it doesn't really matter, but it should be the white wire. Hot (black) goes to the black wire on the switch. Use at least 14 Ga for everything. I use a tall CFL as the top bulb so it's not a lot of power, so stranded 14 Ga is good and easy to work with. The cap is just carbon fiber vinyl over a printed part. The shaft on the switch is 1/4" OD, with 5mm distance on flats (I know, I know). The switch has an 8mm throw, and your cap should only go down 3mm onto the shaft of the switch. The base is just sanded and primered until smooth, then covered with Rustoleum brown sand paint to give it a dirt look. After two hours I went back over it with 4 light coats of Dead Flat Clear Coat (also Rustoleum) to even out the sand roughness and almost give it a cast look.
