A buddy of mine needed a spacer for his extruder fan on his Anet A8 3D printer, so I whipped one up in OpenSCAD and figured I'd share it with the community in case anyone else is in a similar bind. The plate fits snugly around a 40mm X 40mm square fan with a 38mm diameter fan, positioning it a comfortable 2mm away from the heat sink.
