Based on this design http://www.thingiverse.com/thing:2026848 I made significant changes to use a larger M5 screw and nut for better support. I used a thumb screw from here http://www.thingiverse.com/thing:1443439 as well. To assemble, you'll need 1x M5 nut and 1x M5 50mm bolt with a hexagon head. The widening cap goes on the bottom of the bolt to allow for some tolerance in fitting it to actuate the switch. If your printer has limited space at the end, like mine does, you may need to dremel the mount for the lead screw slightly and move it over a bit. I also had to adjust the extruder motor and Z-axis motor mount at the top to keep everything aligned. After making these adjustments, this design worked perfectly.
