I decided to give the gears some extra space from the extruder motor by rebuilding iamburny's herringbone gear extruder with a raised nose design. First, I reversed both gears' bed side so that I could attach the 1mm spacer nose on top. A small clean-out of the Z home might be needed as well if it gets too tight around the hex head hole.
