This is a housing to hold the electronicss (XL6019 based boost converter) of an LED conversion for an Amerian Optical Series 10 microscope. This replaced the 17w tungsten bulb/socket of a 1036 or 1036A illuminator. https://www.microbehunter.com/microscopy-forum/viewtopic.php?f=12&t=18398 The provided plate might work as-is, with a thin chunk of material for the heatsink to rest on to isolate it from direct contact with the plastic, but it is untested. The provided plate is as a reference. The two mounting holes are 57.5mm apart, the center hole for the clip is 12mm back from the centerline of the two mounting holes. There's a second, thinner "plate" which is a template with 2mm holes for marking shape and where to drill holes.
