Pretty much a straightforward copy of the Newmen TC adapter because that's the simplest and most elegant solution I've seen. The Newmen adapter is inexpensive and such a nice fix that I'd suggest buying it from them and giving them support. If that's not feasible, here's a printable version. Print it out nice and coarse (I used a 0.3mm layer height) and stick it to some double-sided tape. Cut the tape to match the shape, then clean the inside of the fork ends and pop these in place. Once they're installed, the hub drops right into place like it would on any normal fork, making installation of the axle much easier. Looks like I was beaten to the punch by Christophe Witte with his model. It looks almost identical, but to be fair, it's not exactly a complicated design.
