I prefer designs that don't need infill or supports. The roller is the only part with infill. I've tried several spool holders, but they all had drawbacks. The bearing ones were difficult to install and remove a spool from. The ones where the spool sat between four bearings sometimes caused too much friction, while the free bearing ones made the filament unroll itself when the roll was almost empty. This design lets you mount the spool without disassembling anything and has just the right amount of grip. I printed the main frame with four walls and enough walls on the rod to make it solid. To print it horizontally, translate the rod by -1 to -1.5mm in the Z direction. There's a 0.3 degree angle between the main arm and the rod, but after about a week of printing, they become square. I glued the rod to the frame and inserted an M3 screw through the top of the rod into the frame.
